How to register

Catchment area

The doctors welcome new patients who live within our practice area.

Due to a large increase in local population with new building development, we are only able to register new patients who live with in our practice boundary.

Please use the map below to check if you live within the practice boundary. You can also check with the reception team.

Medical treatment is available from the date of registration.

How to register

For each application and please make sure you fill in all sections that apply to you. Please make sure you enter your details correctly and double check before submitting the form. The form is also available from the surgeries.

Only use these forms if you are registering as a Regular (permanent) Patient. If you need to be seen as a Temporary Patient please contact reception.

We are mindful that registering at a new practice can be daunting and that there can be confusion as all surgeries have their own way of doing things.  Please read this quick guide for information that is specific to how we operate at Stoke Gifford and Conygre Road Medical Centres.

If you move outside our catchment area

If you move to an address outside of our catchment area, we will ask you to register at a local practice.  After 6 weeks, we will deduct you from our list.

The only exception to this would be where a patient has exceptional circumstances and where referrals have been made and care is yet to be transferred to the hospital and continuity in the short term is important.  An appeals process is not available for out-of-area registration denial, as it is aligned with NHS guidelines.  This policy will be periodically reviewed to ensure its continued alignment with NHS rules and guidelines.
